Insurance

I am in network with Aetna, United Healthcare, Oscar and Oxford in New York, New Jersey, and Florida. I do not accept Medicaid and Medicare at this time. Acceptance of your specific insurance plan will be verified by Headway before your appointment. Should you have out of network mental health benefits you may be eligible to be reimbursed by your insurance provider. I will provide you with a super bill invoice.

I highly recommend that you contact your insurance provider to be informed about your mental health coverage, copayment out of network coverage and deductible information.

Cancellation

Cancellation of your appointment occurring within 48 hours will incur a fee of your session rate. In the event you are unable to keep your appointment you may request a re-scheule based on availability during week of your appointment. If this is not possible, you are responsible for the full cost of your missed session.

Payments

A credit card is required on file for all clients. Payments are due on the date of service and are automatically charged to your credit card. Accepted payments: Visa, Mastercard, American Express, HSA and FSA
